 1. Before project delivery commences

Qualification Certification and Training Declaration

A Qualification Certification and Training Declaration must be completed for all staff and contractors delivering Individual Case Management or Structured Mentoring to participants.

Completed declarations must be emailed to DSD.fundingagreements@sa.gov.au prior to the commencement of any service delivery.

Marketing, Promotion and Publicity Instructions

Providers must comply with the Marketing, Promotion and Publicity Instructions when developing advertising materials or promoting their project.

The Instructions provide direction to providers in relation to requirements for all advertising and promotional materials, including websites and social media; logos, including use and technical specifications; and contact details for media advice and assistance.

JFEP Employer Flyer

The Employer Flyer has been prepared as a promotional tool for providers during preliminary discussions with employers about Jobs First Employment Projects. The flyer outlines the benefits for employers some examples of ways in which employers can be involved.

2. Enrolling participants in a project

JFEP Eligibility Checklist

Providers must verify an individual’s eligibility before they can participate in a Jobs First Employment Project. Participant eligibility is outlined in Appendix A of the Guidelines for Jobs First Employment Projects 2017-18.

Providers should use the Eligibility Checklist to determine eligibility with prospective participants.

Upfront Assessment of Need instructions and templates

Providers are responsible for conducting an Upfront Assessment of Need (UAN) with all prospective participants. The assessment must take place before an individual can participate in a project.

Where accredited training is delivered, the UAN is undertaken in partnership with the nominated Registered Training Organisation(s).

The UAN process is outlined in the Upfront Assessment of Need Working Instructions.

Employment Services Referral Form

The UAN process includes completion of an Employment Services Referral Form if an individual is registered with an Employment Service Provider.

Participant Commencement Form

Each participant must complete and sign the Participant Commencement Form before they commence project activities.

Information collected through the Participant Commencement Form must be entered by the provider into the Skills and Employment Portal. See Skills and Employment Portal Reporting Instructions for further information.

The completed consent declaration pages for the ‘Collection and use of personal information’ must be electronically scanned and submitted to DSD.fundingagreements@sa.gov.au within the timeframe specified in the Employment Projects Service Agreement (EPSA).

3. During project delivery

Skills and Employment Portal Reporting Instructions

The Skills and Employment Portal Reporting Instructions assist providers to report on project participants and activity, and submit claims for payment.

The Skills and Employment Portal is the primary reporting tool for 2017-2018 Jobs First Employment Projects.

4. Completion of project

Participant Exit Form

A Participant Exit Form must be completed and signed by each participant upon their completion or withdrawal from a 2017-18 Jobs First Employment Project.

Service providers are required to enter participant exit information into the Skills and Employment Portal. Service providers are not required to submit hard copy or scanned Participant Exit Forms to DSD but must retain the forms for audit purposes.

Final Report

The EPSA requires providers to review projects upon completion, and document this in the Final Report.

DSD will email a project specific Final Report template to you for completion after the project completion date. An indicative template is available below for your information. 

Financial Acquittal Report

The EPSA requires providers to complete a Financial Acquittal Report for funding received upon completion of a project.
